Monday to Friday: 8.30 - 4.30
Appointments Consultation is by appointment. Urgent cases will be seen on the day. Long appointment If you have a complex health problem, require counselling for emotional support or require an insurance/work medical please book a long appointment. Telephone In most instances a doctor will be available during the day to respond to your telephone enquiries. They may not always be available at the time you call, however a message will be taken and your call will be returned as soon as possible. Home visits If you have a condition which prevents you from attending the surgery and you require a home visit, please contact the surgery as early as possible. This will enable staff to arrange an appropriate time for the doctor to attend. Fees All pensioners, Veterans Affairs patients and those 20 years and under will be bulk billed. All othor patient's fees will be determined at the discretion of their doctor and may vary according to the time spent and items discussed at your appointment. Individual doctor consultation fees may also vary. Payment of fees will be required at the time of consultation. We offer Medicare Easyclaim whereby your claim can be submitted electronically from our practice to Medicare and a refund transmitted directly back to your savings/cheque account. Please talk to your doctor if you are having difficulties paying fees. Emergency after hours care In cases of emergencies and after hours care (including home visits) please contact the Denmark Health Service on 9848 0600 who will contact the on-call doctor to determine what care is deemed necessary and appropriate. |
Referrals
The doctors in this practice are competent at handling all the common health problems. When it is necessary, they can refer patients for further specialist investigation. Please discuss this with your doctor. Prescriptions Prescriptions may be requested without an appointment only if you have had an appointment in the previous three months and the doctor has previously prescribed the medication. A gold coin donation is requested to be placed in the Hospice box on collection of your prescription. Transfer of medical records We are happy to transfer relevant medical records to another doctor but to ensure compliance with privacy regulations we require a signed patient permission and for the requesting doctor to specify which records are required. This ensures that only the right information is transferred, according to the patient's wishes. Patient reminder system Our practice is committed to preventative health care. We may send you reminder notices for routine screening tests and other health services which help you and your doctor monitor your health. If you do not wish to be part of this system please inform the practice nurse; however, please note that if any tests or investigations are abnormal we are obliged to contact you for recall purposes. Patient feedback We value your feedback, both positive and negative - you are most welcome to make use of our suggestion box. Alternatively, you may address issues with your doctor or our practice manager by requesting to see them. Privacy of health information Your medical record is a confidential document. This surgery adheres to current privacy legislation in the collection, use, disclosure and storage of all personal health information. |
